Community Disability Service

The Community Disability Service (CDS) provides holistic assessment and rehabilitation for adults aged 16-64 with complex neurological conditions (excluding stroke) living in Newham.

The specialist multidisciplinary team works together in an integrated way to provide the best standards of care for patients, aiming to restore or maintain optimal health, function, independence and quality of life.

The CDS will see people:

  • Aged 16 – 64
  • Who live in Newham
  • Have a neurological condition (excluding stroke)
  • Need support from at least two health professionals in the CDS
Who can refer?

Anyone, including individuals and their carers can refer to the CDS. This can be done by telephone, fax or referral form. Following an assessment a care plan is developed; providing there are clear rehabilitation or condition management issues to be addressed.

Areas CDS offers support with:

  • Improving mobility at home and in the community
  • Managing spasticity
  • Fatigue management
  • Emotional issues
  • Improving communication/speech
  • Management of swallowing difficulties
  • Managing day to-day activities such as personal care and domestic tasks
  • Returning to work or continuing to study
  • Participating in leisure activities
  • Coping strategies
  • Cognitive impairment
  • Assistance with budgeting
  • Medicine management
  • Skin integrity and wound management
  • Continence management
