Community and intermediate care physiotherapy services

This is a specialist service provided to adults and elders in Newham. We are part of a wider Multi-Disciplinary Team. These services can be provided to people in the following categories:

Community physiotherapy rehabilitation services

For patients between 16 and 64 years with mobility, functional problems and respiratory problems excluding neurological conditions and/or history of falls and patients over 64 years old who can be catered for in the Community.

Home rehabilitation services:
  • Cazaubon unit:
    This enables clients, following discharge from the Hospital, to have ongoing functional rehabilitation for mobility/ functional problems for up to six weeks prior to being discharged to their home environment.
  • Day hospital:
    For patients over 65 years old who require ongoing physiotherapy input in an outpatient setting within specialist groups such as the OTAGO group, a Specialist Falls clinic, Tai Chi group and Chair Based Exercise which are to rehabilitate patients with coordination and balance challenges.

Patients with complex/multi pathologies have access to specialist equipment in the day hospital.

The Service aims to respond to clients (patients) needs faster, prevent avoidable admissions, facilitate timely discharges and improve liaison between all sectors of Health and Social Care.

This service has an open access system, which accepts referral from all agencies including self referrals.
