  • If you are interested in obtaining copies of your own patient notes, please contact the medical records department of the site you have been visiting.

  • If you have any queries about job applications, work experience, or placements then go to www.nhsjobs.co.uk  or contact the Human Resources Department on 020 7655 4000
  • Press enquiries should be made to the Head of Communications on 020 7655 4066
  • All other specific enquiries should be entered into the form below and submitted to the Freedom of Information Act Co-ordinator.
